To refer a patient please call us at (740) 732-7201 and fax orders to (740) 732-2377. A Customer Service Representative will assist you and ask for the following information:
- Patient Name
- Patient History
- Personal Information
- Prescription
- Physician Information
- Insurance Information
- Diagnosis & Statement of Medical Need
After Braden Med Services has received all information from the physician and patient then we can verify benefits from the payer. Once this is in place we can provide the patient with the prescribed equipment and services.
Braden Med Services accepts referrals from most payers, including Medicare, Medicaid, Private insurance plans, Workman's Comp, and private pay. Our Professional Billing Staff work with the patients and their families prior to discharge and throughout the length of the service to provide awareness about insurance coverages.
